Forty years ago, Small Smiles was founded on the principle that all children should have access to high-quality dental care. We remain committed to that principle today. This year the 68 Small Smiles Dental Centers in the US will provide dental care to more than 500,000 children in underserved and lower-income communities nationwide.

Small Smiles is managed by its parent company Forba Dental Management of Nashville, Tennessee.

In January 2010, the US Justice Department settled False Claims Act allegations against Forba Dental Management, the parent firm of Small Smiles. Under the settlement Forba Dental Management will pay $24 million plus interest to the US and three participating states, Maryland, Virginia and South Carolina. The US Justice Department found that Forba Dental Management was liable for:

“… causing the submission of claims for reimbursement for a wide range of dental services provided to low-income children that were either medically unnecessary or performed in a manner that failed to meet professionally-recognized standards of care. These services included performing pulpotomies (baby root canals), placing crowns, administering anesthesia (including nitrous oxide), performing extractions, and providing fillings and/or sealants.”

Small Smiles and Forba Dental Management still faces pending suits related to these issues in the United States District Court for the District of Maryland, the United States District Court for the Western District of Virginia, and the United States District Court for the District of South Carolina.
