Appointments are taken during clinic hours. When making your appointment, please be specific about the reason for coming in, so that we may schedule adequate time. Please have your insurance information available. Please bring all current medications with your to the clinic at the time of your appointment. If you should be unable to keep an appointment, please notify the clinic as soon as possible.
Please register at the reception desk each time you visit the clinic. Please present all insurance identification cards to the registration clerk. Co-pays are due at the time of your visit. Failure to identify your insurance could result in out-of-pocket expenses for you. After registering, you will be directed to the appropriate location within the clinic for your visit. You should notify the ward clerk at the location of your arrival.
