Description
Since 1980, Coordinated Care has been the one-stop source for Western New Yorkers dealing with long-term care issues. As a not-for-profit human services agency, Coordinated Care exists to provide our community with critical support during trying times.
Through the generous contributions of our supporters, foundations, hundreds of volunteers and the United Way, Coordinated Care has become the center of expertise for long-term care in the region. In the coming years, we will continue to offer research, referrals and coordination of services to all those in need – making the challenges of long-term care a little easier to deal with.
Special thanks to the Margaret L. Wendt Foundation for the development of this website. Their generous support enables the elderly, the disabled and their caregivers to understand long-term care and the services available in the community.
