Quality measures are used to gauge how well a hospital provides care to its patients. Measures are based on scientific evidence and can reflect guidelines, standards of care, or practice parameters. A quality measure converts medical information from patient records into a rate or percentage that allows us to assess our performance. Now, even though we may excel in areas such as cardiac care, hospitalist medicine, and others, if we do not document appropriately, it is as if we did not adhere to the guidelines. Second, we have to document that we did so.
