Quality measures are used to gauge how well a hospital provides care to its patients. Measures are based on scientific evidence and can reflect guidelines, standards of care, or practice parameters. A quality measure converts medical information from patient records into a rate or percentage that allows us to assess our performance. Now, even though we may excel in areas such as cardiac care, hospitalist medicine, and others, if we do not document appropriately, it is as if we did not adhere to the guidelines. Below is a list of these Core Measures. When you look to see how they are stated, you can see that the "small stuff" becomes quite important... First, we have to do right by the patient. Second, we have to document that we did so. Become familiar with these measures and don't be afraid to point out documentation deficiencies to the physicians, NPs, and PAs if you happen to notice them. Likewise, no hard feelings if someone points out deficiencies in your documentation...
