The following insurance benefits are provided to all active members who are enrolled for 30 or more consecutive days in the Protective SmartHealth Plus & Premier programs as added separate benefits at no additional cost to members†.:
† The insurance benefits are underwritten by an A.M. Best rated insurance company and subject to the exclusions, limitations, terms and conditions of coverage as set forth in the description of coverage provided in your membership materials and the policy issued to Protective Marketing Enterprises, Inc. Coverage is not provided for loss due to a pre-existing condition for 12 months from the covered person’s effective membership date. These benefits are provided at Protective Marketing Enterprises’ expense to all active members who are enrolled for 30 or more consecutive days in the program as added separate benefits at no additional expense to members.
- Savings Source: Physician savings based on actual discounts off participating provider’s normal retail charges (National average discount 21%). Dental savings based on Protective’s 200 Series Dental Fee Schedule for participating general dentists (fees vary by region) compared to the 2004 National Dental Advisory Service National Average (participating specialty dentists provide a discount of 20% off their normal retail charges). MyCL savings based on actual discounts off retail prices. Vision, Prescription, and Chiropractic savings based on actual discounts off participating provider’s normal retail charges. Actual savings may vary.
