I-Context of Health Information System Evolution in Occidental countries
I-1- Stakes in the control of healthcare spendings
There is a lot at stakes in the control of healthcare spendings in Western countries today with:
-the aspiration of everyone to a better living,

factors that have all the effect of increasing healthcare expenditure in the national budgets.
Maintaining the same growth rate in healthcare demands come up against certain limits, which are set by the available financial resources
They are today achieved and even exceeded in certain countries like France.
The control of these healthcare expenses becomes an absolute priority for countries willing to maintain a high medical level and even to optimize healthcare offers in order to face the increasing needs in this field.
The search for productivity gains has been actively started in France like in most of the European countries with the starting in the last three years of:
-medical references become indispensable to doctors in their prescriptions to patients (ref. Medical References Obligatory in France),
-new Hospital Financing Systems orientated to Stays Pricing, variable according to the Stay being part of the 800 Pathologies : the Diagnosis Related Groups (the latter having inspired the name of PROVALCARE).
-new Health Information System supported by Health Cards for the patient administrative identification and the follow-up of their medical profile, as of the Social Health Network implemented in France since 1998.
I-2- Stakes of the implementation of Health Information System focused on the patient
Control of healthcare spendings impose implementation of Health Information System focused on the patient. Implementation of these new Health Information Systems is today planned inside the hospitals and on a national and international level.
Inside the hospital, this target is achieved by the implementation of the medical file and the nursing file.

The implementation of these new tools must especially permit the mobilisation of the productivity gains that exists in the running of Hospitals.
As an example : in France, enormous productivity possibilities up to now unsuspected exist. Likewise if we take into consideration the structure of hospital costs and the global cost of the functioning of Public and Private Hospitals, the stakes related to an optimisation of hospital expenses exceed 8 Billion €.
